Primary Care clinic provides comprehensive holistic clinical services to Cherokee people of all ages. Health care is delivered in a team based setting and includes medical, nursing, pharmacy, nutrition, patient registration and behavioral health care. Care is delivered through an open access model and appointments can be made for same day and long term needs. We focus on both your same day needs and the best medical practices to ensure a healthy life.
Cherokee Indian Hospital is recongized as a level 3 Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA). Level 3 is the highest level of recogintion you can receive in this category and and is an innovative program for improving primary care. There are specific sets of standards that must be met to show that your practice is organizing care around the patient, working in teams and coordinating and tracking care over time. Cherokee Indian Hospital sought recoginition as a Patient Centered Medical Home in hopes of strengthening the patient-provider relationship by replacing episodic care based around illness and patient complaints with coordinated care and long-term team based relationships.

1) How do I get a primary care provider if I don’t have one?
Our preference is that you choose one of the providers who is open to receive new patients or one will be chosen for you. If there is a conflict you can change your PCP up to two times a year. Forms can be obtained at the Primary Care front desk.
2) How do I make an appointment?
Please call your primary care providers direct phone number for the team. Someone will be happy to schedule an appointment. If no one answers, we request that you leave us a message including your name, chart number/Date of Birth, reason for the call and a phone number where you can be reached. We will make every effort to return your call quickly and efficently.
Extended Primary Care appointment hours are now available after 4 pm the second Wednesday of every month.
